What to bring with you at the time of your first appointment
- For New Patients– Please complete the new patient forms that were mailed to you and return it with the postage paid envelope enclosed, prior to your initial appointment
- A list of all of your current medications
- Insurance cards
If your insurance is not one we participate with, payment is due at the time of service. We accept cash, check, Visa, Mastercard and Discover.
Some insurance companies require a referral from your Primary Care Physician to see a specialist. You will need to check with your insurance company or Primary Care Physician to determine if this is necessary. Without a referral you will be asked to sign a waiver of understanding that your insurance company may not cover your visit.
Children are delightful patients. They make up a special segment of our practice. We try to ease normal anxieties about a visit to the doctor by providing a cheerful office setting.

We participate with Medicare, Martins Point, Anthem Blue Cross & Blue Shield and Community Health Options.
We may submit your claim as a courtesy if you have a Medicare Advantage plan other than what is listed above. Please call our office for details.
These will depend on the size and the complexity of the procedure. The excised lesion will be sent to a specialized lab for further testing. The lab will have separate fees and you may receive a bill directly from them.
Some insurance companies do not pay for second opinion without prior authorization. Please check with your insurance company before calling to schedule an appointment.
Dr. Jensen will provide you with enough refills to last until the next appointment. If additional refills are needed, please plan ahead as most prescriptions take at least 24 hours to fill. If it has been longer than 1 year since your last visit, you will need to be seen before a refill can be given.
